The legal representative of a patient who is a minor (person with parental authority) or an incapacitated adult patient (curator or guardian), including those with a legally instituted mandate in case of incapacity.
A temporary representative of an incapacitated adult patient (spouse or, in his or her absence, a close relative; agent with a unofficial mandate in case of incapacity; or any other person expressing a specific interest in the patient).
The legal representative or power of attorney of a capable adult patient.
The heirs or legal representatives of a deceased user.
You may also seek assistance from a person of your choice.
Any citizen who witnesses a situation he or she considers a threat to the integrity or safety of an individual or group of individuals, or who believes that reprisals may be taken against him or her may report the situation to the Québec Ombudsman.
